And the histogenesis of the colorectal carcinoma was discussed and the relashioship between the development of the colorectal carcinoma, and the continous intake of high-fat foods could be pointed out from the investigations using human cells and organs, for the first time.422Juntendo Medical Journal2021. 67(5), 422-426Special ReviewsHistogenesis of Colorectal Carcinoma Considering from Clinicopathological IntroductionThe pathologists like me sometimes observes Paneth cell (Pa) in the colorectal epithelial neopla-sias (CEN) (colorectal adenoma (CRA), colorectal carcinoma (CRC)) (Figure 1)1-3).Pa is usually situated at the base of the glands of the small intestine (Figure 2), were first identified by Scwalbe4) in 1872, were studied in detail morpho-logically by Paneth5) in 1888, and is also seen the non-neoplastic mucosa6, 7) in the inflammatory bowel diseases, for example ulcerative colitis and this findings are called as the colorectal Paneth cell metaplasia (CR-PaM).As the reason of the appearance of Pa in CRA, which has been the pre-condition of the develop-ment of some CRC8, 9), is unclear, I have examined the incidences or general features of Pa in the CRA or the non-neoplastic colorectal mucosa in non-in-flammatory bowel diseases for many years, and some findings described below have been detected10, 11).In the current review, these findings are showed and the histogenesis of the development of CRC Key words: Paneth cell metaplasia, colorectal cancer, colorectal adenoma, Paneth cell, K-ras mutationDepartment of Pathology, Juntendo University Shizuoka Hospital, Shizuoka, Japanconsidering from these findings is expressed as my opinion.1. Colorectal adenoma with Paneth cell (CRA-Pa)In 1980’s when the incidence of CRC in Japan was lower than that in United States America (USA) (that in Japan : that in USA = 1 : 4), I have studied the incidence of CRA-Pa in Japan10), and in Kuakini Medical Center at Hawaii, USA11).The results of these studies revealed that the incidence of CRA-Pa in native Japanese was lower than those in Japanese descendants and Caucasian residents in Hawaii, and this finding was thought to show that the incidence of CRA-Pa is related to the lifestyle of European and American. And really, in the present time (in 2018) when the lifestyle of European and American has been established, the incidence CRA-Pa in native Japanese was increased and equal to those in Japanese descendants and Caucasian residents in Hawaii or United States America in the 1980’s (Figure 3).
